I know this is an old post, but ADs do exist, they are not user-caused, but caused by mechanical failures of the firearms; users cause NDs.

Although what follows in the following video is very rare, this dear gentleman’s riveting story is compelling. Notice how this gentleman has a mask in his still broken hand, so it’s pretty recent. Some things to be gleaned from it.
I was at the range myself recently, only recently decided to bring along a first-aid kit in me range-bag, ended up injuring my hand (minor injury), used my kit, and was able to keep on going - enjoying my practice time. Range-Safety-Officer (RSO) showed me his five scars on his hands, and we had a laugh about it all. Still healing today, but I’ll be back to 100% in 1-2 weeks. Stay safe all.
