Seriously, the only thing I can think of is the chamber was out of sync with the barrel. Don’t laugh at me, I have very little understanding of the operation of revolvers.
That or shot straight into a squid I mean squib?
I was thinking squib lodged in the forcing cone. If it lodged further down the barrel you probably would see a barrel bulge or break, but in the forcing cone would send all that pressure straight back into the cylinder. That, or maybe a way over-pressure hand load?
I had a squib once lodge directly between the cylinder and forcing cone, but that locked up the action so there was no chance of firing another one into it.
